WisCaregiver Career Program CDCP Agreement

All boxes below must be checked as a requirement to register for the WisCaregiver Career CDCP Program. 

*HCBS Provider Definition:

Home and community-based services (HCBS) providers contract with Medicaid so Medicaid beneficiaries can receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and the elderly. Funding to support your free training, competency testing, and financial incentives comes from the American Rescue Plan Act (ARPA) and requires individuals receiving HCBS services to benefit.
