Gateway to Direct Support Professional (DSP) Credentialing Agreement

All boxes below must be checked as a requirement to register for the Gateway to DSP Credentialing Program.

Agrees to share information and allow information sharing with University of Wisconsin Green Bay and the Gateway to DSP Credentialing team for purposes of the Gateway to DSP Credentialing Program.

Agrees to successfully complete the Gateway to DSP Credentialing Program.

Agrees to schedule competency test within one month of completing training.

Agrees to successfully complete competency testing within two attempts.

Agrees to continue employment or apply for employment with Gateways, Opportunity Networks or Nashua Center within one month of completing the competency testing.

Agrees to participate in a secure online survey at the conclusion of my participation in the Gateway to DSP Credentialing Program.

Agrees to meet with an assigned mentor (via phone or video call) at least three times as a requirement to receive the student bonus while engaging in the CDSP program.

Agrees to keep contact information up-to-date by contacting the Help Desk (NHDSP@uwgb.edu)

I acknowledge that I will receive the following bonus series upon completion of the credentialing program and successful passage of the exam:

$1,000.00 bonus (while grant funds are available)

$250.00 bonus for Alvirine students .
