WisCaregiver Career Program CDCP to CNA Ladder Program Agreement

As part of the WisCaregiver CDCP to CNA ladder pilot you agree to complete the CDCP certification by 6/30/2025 and the CNA certification (training and testing) by 9/30/2025.

WisCaregiver Career Program CDCP Agreement

All boxes below must be checked as a requirement to register for the WisCaregiver Career CDCP Program.

Agrees to share information and allow information sharing with University of Wisconsin Green Bay and Wisconsin Department of Health Services (DHS) for purposes of the WisCaregiver Career CDCP Program

Agrees to successfully complete the certified direct care professional (CDCP) program.

Agrees to schedule competency test within three months of completing training.

Agrees to successfully complete competency testing within two attempts. I understand that if I need to take the competency more than two times to achieve a satisfactory score that I will have to pay for the additional tests.

Agrees to continue employment or secure employment with a home and community-based services (HCBS) provider/agency within two months of completing competency testing (see definition of a HCBS provider/agency)*.

Agrees to work full-time or part-time in a participating home and community-based services (HCBS) provider/agency for at least six months in exchange for CDCP training and testing at no cost to me (full-time and part-time as defined by HCBS provider)

Agrees to participate in a secure online survey at the conclusion of my participation in the WisCaregiver Career CDCP Program

Agrees to participate in a secure online survey prior to my 3-year recertification in the WisCaregiver Career CDCP Program.

Agrees to keep contact information up-to-date by contacting the Help Desk (cdcp@uwgb.edu)

I understand that receiving the $250 sign on (stay on) bonus and retention bonus is contingent on working at a participating WisCaregiver Career CDCP HCBS provider/agency. I understand that it is my responsibility to inform my employer that I have completed the CDCP program and testing and that my completion information can be found on the CDCP registry. If funding that supports the bonus program ceases, DHS will inform all individuals on the CDCP Registry.

*HCBS Provider Definition:

Home and community-based services (HCBS) providers contract with Medicaid so Medicaid beneficiaries can receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and the elderly. Funding to support your free training, competency testing, and financial incentives comes from the American Rescue Plan Act (ARPA) and requires individuals receiving HCBS services to benefit.
