If you become employed as a caregiver for a Wisconsin Medicaid Recipient, you may be eligible at that time to receive your bonus.

CDCP to CNA Career Ladder Course – Participant Attestation
Please read the statements below carefully. By clicking "I understand" in this attestation, you acknowledge that you understand and agree to the requirements of participation in the Certified Direct Care Professional (CDCP) to Certified Nursing Assistant (CNA) Career Ladder Course.
Program Understanding:
  • Upon successful completion of the CDCP Certification, I may enroll in an in-person CNA course through the Healthcare Workforce Training Institute at the reduced rate of $750, which my employer or myself will be responsible for paying during the class registration process.

  • I understand that after successfully completing the in-person CNA course, should I wish to take the state tests to become a certified nursing assistant, my employer or I will be responsible for paying for these.
I understand that by participating in the CDCP-to-CNA Career Ladder Course:
  • I will receive FREE access to the CDCP Course and CDCP Certification.
  • Upon successful completion of the CDCP Certification, I may enroll in an in-person CNA course through the Healthcare Workforce Training Institute at the reduced rate of $750.
  • Participation in the CDCP Course will result in a reduction of approximately 20 hours of in-person CNA class time.
  • Completion of this Career Ladder Course may make me eligible for monetary bonuses, depending on my employer and length of employment.
  • The CNA portion of the course is held in-person only at Healthcare Workforce Training Institute Education Centers.
  • Education Centers are located in Bloomer, Chippewa Falls, Eau Claire, Fennimore, Lodi, Osseo, Rice Lake, Spooner, St. Croix Falls, and Superior.
  • There are no Education Centers currently located in Eastern Wisconsin or the Milwaukee area.
  • I am responsible for arranging my own transportation to attend in-person classes.
  • After passing the CNA class entry quiz, I will have 180 days (6 months) to enroll in and complete the CNA course.
  • Failure to complete the CNA course within this timeframe will result in dis-enrollment from the CDCP-to-CNA Career Ladder Course.
  • If dis-enrolled, I will no longer be eligible for the reduced CNA course rate or reduced in-person class time.

WisCaregiver Career Program CDCP Agreement

All boxes below must be checked as a requirement to register for the WisCaregiver Career CDCP Program. 

*HCBS Provider Definition:

Home and community-based services (HCBS) providers contract with Medicaid so Medicaid beneficiaries can receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and the elderly. Funding to support your free training, competency testing, and financial incentives comes from the American Rescue Plan Act (ARPA) and requires individuals receiving HCBS services to benefit.
